Patients will need to register with our office for your initial visit with Dr. Kundi. This can be done in one of two ways:

  1.  You may register through our secure patient portal online or Y
  2. You may download our patient registration forms, complete them and then bring them in to the office with you for your initial visit.

03.

Financial/Insurance

We accept most major health insurance plans. If we are participating with your health insurance plan, we will file your insurance for you. Please be prepared to pay any co-payment, deductible or non-covered services on the day of your appointment. For your convenience, we gladly accept cash, checks, debit and credit cards.

We ask that you allow at least 48 hours for prescription refills. This gives Dr. Kundi time to review your medical chart to determine if a follow-up appointment is needed or if a refill can be called in to the pharmacy. When calling, please provide us with the following information:

  • Name of your Pharmacy
  • Pharmacy phone number
  • Name and dosage of your medication

Please understand that, as a general rule, we do not phone in prescriptions after hours or on weekends.
